  • Exercise on Referral

    Exercise on referral (EOR) supports people with medical conditions (who are not normally active) to access a supported exercise programme (normally for 12 weeks) with the help of a specialist adviser.  The majority of these take place in local leisure centres and community venues.

    If the face-to-face meetings offered by Slimming World aren’t for you, this app, tablet or computer-based 12-week programme may better suit your lifestyle.

    You can choose your preferred weight management plan from options including digital support and 1:1 coaching sessions.

    It is available to people who are 18 years and over and
    – Have a BMI of 30 or more, or 27.5 for people from black, Asian and ethnic minority backgrounds
    – Have diabetes or high blood pressure
    – Can access a smartphone or tablet or computer, and have an email address

  • NHS Diabetes Prevention Programme

    The NHS Diabetes Prevention Programme (NDPP) is a 9-month programme to build consistent lifestyle changes for those diagnosed with pre-diabetes.
    Eating well, moving well and taking charge of your health.
    You can participate in face-to-face group sessions or virtually. The first 6 sessions are fortnightly and the remaining sessions are monthly.

    You can sign-up if you are aged 18-75 and:
    – Have had an HbA1c blood test in the last six months with a result of 42 – 47 mmol/mol
    – Or a previous diagnosis of gestational diabetes
    – Not currently pregnant

    The NHS Diabetes Prevention Programme story

  • Type 2 Diabetes Path to Remission

    The NHS and its partner Diabetes UK are delivering a new programme which provides a low calorie, total diet replacement treatment for people who are living with type 2 diabetes and obesity or overweight.
    You can be referred to this programme if you:
    – Are aged 18 to 65 years
    – Have been diagnosed with Type 2 Diabetes within the last 6 years
    – Are not currently using insulin
    – Have a BMI of 27 or more, or 25 for people from black, Asian and ethnic minority backgrounds
    – Have had an HbA1c blood test in the last 12 months with a result of 43-87 if you are taking diabetes medication, or 48-87 if you are not taking diabetes medication
    – Are not pregnant or breastfeeding
    – Do not have some other illnesses (ask your diabetes nurse)
    Ask to speak with your diabetes nurse if you would like to be referred.
