Accessing your health record

Your health record includes information like any conditions or allergies you have and any medicine you’re taking.

Most patients will automatically have access to more information added to their health record. This includes letters, test results and appointment notes.

You can access your health record and nominate someone you trust to access it too.

You can use your NHS account (through the NHS website or NHS App) or SytmOnline to access your health record.

You’ll need to ask the surgery for online access to your full health record, or you’ll only see your medicines and allergies.

Request access to your full health record online

The Access to Health Records Act 1990 and Data Protection Act give patients/clients/staff or their representatives a right of access, subject to certain exemptions, to their health records. Stow Surgery respects the rights of individuals to have copies of their information wherever possible.

Personal information collected from you by this form, is required to enable your request to be processed, this personal information will only be used in connection with the processing of this Subject Access Request.

Charges payable

In accordance with legislation, no fee will be charged for your request, unless the request is manifestly unfounded or excessive, particularly if it is repetitive. Before any further action is taken, we will contact you with details of our “reasonable administrative charges” in order to comply with your request.
