Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access.

Adults and Young People Over 16 years:

The patient needs to come into the surgery in person and complete a proxy access form.

Children Aged 13 to 15

The NHS App is available for use by anyone aged 13 or over who is registered with an NHS GP practice in England.

No proxy access is granted to parents/carers / guardians of children in this age group unless there are exceptional circumstances.

Children Aged 13 and Under

At Stow Surgery proxy access to medical records will be granted to parents / guardians / carers in exceptional circumstances only i.e. children with multiple medications and serious medical problems, who may never develop capacity to manage their medications and health records themselves. 

Those with parental responsibility can request proxy access for medication ordering only where the patient has one or more regular repeat medications.  The request must be made in writing (using the Proxy Access request form) and access will be reviewed on a case-by-case basis.  Proxy access for medication ordering will be reviewed every 12 months in line with the medication review.

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.
